PET - Osteosarcoma

19-year-old male with history of osteosarcoma of right lower leg.Follow-up CT scan showed growing lesion in lung. Leg biopsy negative for residual tumor.Whole-body PET scan showed high FDG uptake in femur. Lung lesion seen on CT had low lung accumulation rate with SUV of 2.Original treatment plan based on CT, and biopsy would have been thoracotomy to resect lung lesion. After PET scan found high FDG uptake in leg, another femur biopsy was attempted but unsuccessful. Two weeks later, patient's thigh exhibited swelling, and another biopsy was done which revealed a malignant fibrous histiocytoma. The leg was amputated.
